Endodontics: creating access and preparing the canal

In many cases, root canal treatment is the only way to save a tooth. The goal is to remove infected and necrotic tissue from the canal system as completely as possible, followed by a dense root filling. Rotary instruments play a central role in this process, above all in gaining access and exposing the canal orifices.

The access cavity

The first step is a precise access cavity. Diamond or tungsten carbide instruments are used to create straight-line access to the pulp chamber without unnecessarily sacrificing healthy tooth structure. A clear view of the pulp floor and locating all canal orifices are essential. Special opening burs make it easier to remove dentine overhangs.

Preparation: reciprocating or rotary

The actual canal preparation is carried out with endodontic file systems. In principle, a distinction is made between:

  • Reciprocating systems: alternating clockwise-anticlockwise motion, often with a single file, robust and with a reduced risk of fracture
  • Continuous rotary systems: even rotation, frequently with a coordinated file sequence

Irrigation and obturation

After preparation, irrigation for disinfection is followed by obturation, i.e. the dense filling of the canal. The techniques range from the single-cone technique to thermoplastic filling techniques. A dense root filling prevents bacteria from re-entering.

Don't forget hygiene

Endodontic instruments are medical devices and are properly cleaned, disinfected and sterilised. With files, the limited number of uses must also be observed in order to avoid instrument fractures.
