Wherever the surface is not smooth. The cup adapts to curvatures, the bristles penetrate into depressions.

Soft near the gingiva and on sensitive surfaces, harder for stubborn deposits on enamel. Hardness determines how much the bristle yields on contact.
Hard bristles work faster but irritate the gingiva and can attack exposed dentine. The principle is the same as with the cup: when in doubt start softer and increase contact time.
Prophylaxis requires paste. The bristle transports the abrasive, it is not an abrasive itself.
Some polishing brushes for the laboratory are impregnated and work without additives. Each item states which applies. Anyone using a plain prophylaxis brush without paste barely cleans and is then surprised by the result.

Brushes produce a natural shine. For a mirror-like gloss on composite or ceramics, multi-step polishers are the better choice.
As soon as the bristles are permanently splayed. A frayed brush distributes paste instead of guiding it.
